How to report accidents and dangerous occurrences
The terms accident and dangerous occurrence are defined in the petroleum legislation and also include near misses and emissions.

- Petroleum (Submerged Lands) (Management of Safety on Offshore Facilities) Regulations 2007 - Part 5
- Petroleum (Submerged Lands) (Pipeline) Regulations 2007 - Part 6
- “Schedule of Onshore Petroleum Exploration and Production Requirements -1991”.
Onshore and coastal waters
Accidents and dangerous occurrences in relation to petroleum, pipeline and geothermal operations must be reported to:
- Department of Mines and Petroleum: Resources Safety Division Duty Officer - (pager) 08 9476 0193 or
- Director Petroleum Safety - 9222 3595 (Office Hours) 0429 201 295 (After Hours)
and
- For reporting significant matters related to well integrity call Mr Steve Walsh 0427 081 863. Follow-up written reports where required to reports.upstream@dmp.wa.gov.au.
Any incidents that have environmental impact must also be reported to the Environment Division Duty Officer - 0419 960 621.
Offshore
Where there is a statutory requirement to report significant offshore incidents to the Executive Director Petroleum Division as well as NOPSEMA relating to:
- well, reservoir or facility integrity which may or do compromise integrity of wells or facilities and/or
- interruptions to petroleum production and/or
- medivac evacuation / fatality.

Where there is a legislative requirement for rapid submission of a brief follow-up written report they should be directed to the Executive Director Petroleum Division at the following email address: reports.upstream@DMP.wa.gov.au.
To report an Environmental incident contact the Environment Duty Officer on 0419 960 621. Further information with regards to Environmental matters can be obtained by clicking on the link located on the right of the page.
Offshore Safety related matters in commonwealth waters should be reported to NOPSEMA on 6461 7090. Further information can be obtained from the NOPSEMA website on the attached link located on the right of the page.
Reporting forms and procedures
Please note:
A monthly status report form must be submitted whether or not there has been a reportable accident during the month.

Safety performance in the WA petroleum industry – statistics
Each month, accident and injury details are reported to Resources Safety by the operators of petroleum facilities and pipelines, as are the number of persons employed (including contractor employees) and the hours worked during the month. The data is provided in an effort to aid the development of injury prevention strategies.
From this data, Resources Safety produces the Safety Performance in the Western Australian Petroleum Industry annual poster, which includes statistics on lost time, alternative duties and medical treatment injuries. To view the annual poster, refer to the publications section.
