Copy of certificate

If a certificate(s) of competency (including an authorised mine surveyor’s certificate) has been lost, stolen or damaged, the certificate holder can apply to receive a copy.

Lodging the application

The completed and signed application form must be accompanied by:

  • all support documents
  • the fee, payable at lodgement
    • this can be paid by credit card
    • application to be lodged online
