Changes were made to the following online forms as a result of the Mining Amendment Regulations (No. 4) 2012 that were published in the Government Gazette on Friday 9 November 2012.

- Form 9 - Extension of Term/Renewal
- Form 16 - Objection
- Form 18 - Application for Exemption
- Form 21 - Application for Mining Tenement
- Form 30 - Application to Amend
Changes to the requirements for tenement advertising - The department is now responsible for advertising application details on the DMP website.
