This page provides information on documentation required for new and revision Health Management Plan submissions.
Mandatory attachments
The previously prepared Health Management Plan AND Health Risk Assessment must be attached.
Click 'Attach File' after each file attached.
Select the 'Type of Documentation' for each file attached.
To delete a file attached in error, click the delete icon.
Supporting attachments
You may attach any supporting files (see examples above). These are optional attachments.
Select the type from the drop down.
Enter 'Description'. This is a free-text entry. E.g. Risk Report - HMP(Reference ID) - Combined Operation name - Date.
Drag and drop attachments
You can drag and drop files in the saved drive into this area using the drop file functionality.
Revision Submissions
You will need to attach the mandatory documents, any supporting documents in support of the revision.
Enter the reason for the revision.
