Copy of certificate
If a certificate(s) of competency (including an authorised mine surveyor’s certificate) has been lost, stolen or damaged, the certificate holder can apply to receive a copy.

Lodging the application
The completed and signed application form must be accompanied by:
- all support documents
- the fee, payable at lodgement
- this can be paid by credit card
- application to be lodged online
